Project Overview
Between December 2016 and August 2018, we are designing, testing and evaluating practical, collaborative models to improve referral pathways between generalist and specialist CLCs, increase their exchange of knowledge and expertise, and, ultimately, ensure their clients receive the expert assistance they need.
This work is being conducted in the following three phases:
Phase One: Planning, research, and sector-wide consultation (Dec 2016 – Sep 2017)
Phase Two: Pilot model testing (October 2017 – July 2018)
Phase Three: Evaluation and reporting (July 2018 – August 2018)
